Keys Leaving Care seeks to support those care leavers who at the age of 16 and beyond are unable to live independently, are particularly vulnerable in the community and whose needs remain highly complex. Our passion is to deliver positive outcomes in line with the Every Child Matters framework, to empower all young people to develop effective life skills and make positive changes that make a real difference.
The Keys leaving care service was registered by the Care Quality Commission on 16th October 2018 to deliver support and care under a regulated activity for young people living in the community, which means that we are now in a position to deliver a wider range of care and support packages. This is in response to the increasing number of referrals that have come to us for young people with more complex needs often requiring higher levels of care, such as administration of medication, which we have had to decline in the past, but are now able to accept.
Undoubtedly the step we have taken will both strengthen governance and ensure ongoing high quality service provision for Young People, whilst ensuring that we are working within Ofsted guidance of what constitutes care versus support.
What we do
Using person centred planning, we aim to bridge the gap between leaving care and living in the outside world, by supporting each young person to maximise their potential and develop the resilience and life skills required to live independently, maintaining their own tenancy in the local community.
We provide personalised programmes for young people, including:
- A wide range of support option for young people, focusing on complex needs with phased reduction plans, transition plans from internal services ad low level support plans with tenancy take over options
- From 12-24 hour support and accommodation options with phased hourly reduction plans
- Bespoke long-term housing provision for each young person
- Early release planned placements for young people in custody
- Packages of accommodation and/or support for parent and child
- Early intervention family support work in the family’s homes
- Ensuring best possible outcomes- e.g. continuing education, gaining employment/training opportunities with support from our NEET Manager
- Sleep in and waking night staff
- One to one independence training
- Specially designed education programme for young people who are no longer in mainstream education
- Implementing a transitional model for young people moving on from a residential setting
- 24/7 advisory on call service 365 days a year
